Position Title
Transitional Care Center: RN Day/Night (PRN - Bellevue Location)
Description
General Summary: The Registered Nurse I (RN) assesses, plans, implements, evaluates and provides nursing care to assigned patients utilizing the nursing process to facilitate the patient’s progression toward optimal health. The RN provides, directs and guides patient/family teaching and readiness for discharge to prepare the patient for self, home or further professional care. The RN is active in department activities, transdisciplinary team activities, and LifeCare activities to ensure individualized, patient-centered health care for all patient populations admitted to LifeCare Hospital.
Patient Population: Delivers direct patient care appropriate to the patients served. The ages of the patients served are adult (18-65 years of age) and geriatric (65 and older).
Essential Functions:
1. Assesses and documents the basic physical, psychological, social, cultural, spiritual, safety and developmental needs of patients and families and communicates changes in patient needs or condition to the supervisor or physician when applicable.
2. Identifies patient care problems/needs; establishes patient outcomes in response to identified needs; plans patient care in collaboration with the transdisciplinary team, and documents and communicates the plan to others.
3. Implements the plan of care in a cost effective manner, demonstrating basic competency and proficiency in nursing interventions and practice. Documents interventions and patient response in a time frame appropriate to the event. Responds appropriately to emergency situations.
4. Evaluates patient health care status in relation to outcomes achieved as a result of nursing interventions and/or interventions by others. The outcome of care provided and progress toward goals are documented.
5. Collaborates with transdisciplinary team members, including the patient and family to identify and meet admission, transfer and discharge needs of patients and families throughout the course of care. Provides education to patients and families concerning diagnosis, treatment, hospitalization and post discharge care.
6. Facilitates admission and transfers of patients including appropriate patient placement. Knowledgeable about the admitting process. Communicates changing patient needs to supervisor/MD. Delegates responsibilities appropriately.
7. The knowledge and skills necessary to perform the position requirements are demonstrated through the successful completion of competencies established for the position, to include population served, and other special needs of patients or customers, served by the department.
8. Adheres to LifeCare policies, procedures, all safety plans and all standards imposed by regulatory organizations.
9. Regular attendance and timeliness is required.

Requirements
Supervision: Oversees and directs delegated functions of LPN’s/LVNs, CNAs, Patient Care Technicians, Mental Health Assistants, Monitor Technicians and Unit Clerks.
Experience: Graduate RN or RN with less than 1 (one) year of experience in an acute clinical setting.
Education: Graduate of an approved registered nurse program is required.
Licensure/Certification: Current Registered Nurse license in the state where practicing is required. Current BLS certification is required upon hire and ACLS certification is required within six (6) months of hire. All Registered Nurses must maintain current licensure, BLS and ACLS certifications while employed with LifeCare Hospitals.
